What is the SADI-S Operation? Why Influencer Remi Bader Chose It. By Edmund B. Chen on April 08, 2025

What Does “SADI-S” Stand For?

SADI-S stands for Single Anastomosis Duodenal-Ileal Bypass with Sleeve Gastrectomy. It’s sometimes called a modified duodenal switch or loop duodenal switch. In our expert opinion, it’s one of the most effective and durable bariatric surgeries offered today, particularly for those with higher BMIs or weight regain after previous procedures.Visual representation of the SADI-S weight loss surgery procedure

How Does the SADI-S Surgery Work?

In this advanced procedure:

  1. sleeve gastrectomy is performed first, where approximately 70% of the stomach is removed.

  2. The remaining stomach (the “sleeve”) is then disconnected from the first portion of the small intestine (duodenum).

  3. The sleeve is reconnected to a loop of intestine about 300 cm from the ileum (the end of the small intestine).

This results in:

  • A powerful restriction of food intake

  • A significant bypass of the intestine (400–600 cm)

  • A drastic reduction in calorie and fat absorption

Why Influencers Like Remi Bader Are Choosing SADI-S

remi bader before and after weight loss surgeryRemi Bader is known for her unfiltered honesty about her body, mental health, and now—her experience with weight loss surgery. She underwent SADI-S to address health concerns and improve her quality of life. Her story resonates with thousands of people who are exploring bariatric surgery options beyond the traditional gastric sleeve or bypass.

Remi has shared that her recovery wasn’t easy, including some side effects and emotional challenges. But her transparency is opening up important conversations about body autonomy, realistic expectations, and the diversity of weight loss journeys.

What Makes the SADI-S So Effective?

Compared to other procedures, SADI-S offers:

  • 70–80% excess weight loss, which is superior to the sleeve gastrectomy or Roux-en-Y gastric bypass (RYGB)

  • Better resolution of Type 2 diabetes, sleep apnea, high blood pressure, GERD, and arthritis

  • Lower rates of marginal ulcers (less than 1% vs. 5–20% with RYGB)

  • No dumping syndrome, thanks to preservation of the pylorus (the stomach’s natural valve)

SADI-S vs. Traditional Duodenal Switch

While both are powerful, the SADI-S is:

  • Simpler and safer (with fewer long-term complications)

  • Less likely to cause severe vitamin deficiencies

  • Equally effective in long-term weight loss

Can SADI-S Be a Second Surgery?

Yes! In fact, many patients—including those with a prior sleeve gastrectomy—find the SADI-S to be an excellent revisional option. It’s ideal for:

  • Patients with BMI > 45

  • Individuals who have regained weight or didn’t lose enough with the sleeve alone

Recovery After SADI-S

We perform the SADI-S exclusively using the Intuitive Da Vinci robotic platform, offering unmatched precision and faster recovery. Patients often report:

  • Less post-operative pain

  • Minimal nausea

  • Quicker return to normal activity—especially for those converting from a previous sleeve

Some patients even say their recovery from a SADI-S was easier than their original sleeve surgery.
